Overview

As a Registered Nurse - Case Manager, you will coordinate patient care, assess and evaluate discharge plans, and ensure treatment plans are initiated and monitored daily. This role involves working closely with patients to facilitate their transition to appropriate care levels, all while adhering to Medicare guidelines. You’ll work Monday through Friday, with no weekend shifts and no on-call requirements.
