Overview

In this role, you will assess and develop discharge plans for patients while coordinating services to ensure they receive appropriate care. You'll initiate and evaluate treatment plans, monitor progress daily, and address any barriers to discharge. The position offers a Monday to Friday schedule with no weekend shifts, allowing for a balanced work-life. Ideal candidates will have at least three years of experience in case management.
